I have proof of delivery as well as a document that the parcel was signed and received by the recipient. How can I attach this document to my chargeback response? (see the image, nowhere available to upload)

Remember, a charge back is between you and the buyer's credit card company. Amazon will only pass thru what you reply with.
In your case, type is out just like amazon showed. Make sure the tracking number is in there along with the dates shown on the tracking.
