Assuming you have the NA Unified account (which you almost certainly do) -- the $39.99 fee covers all the North American accounts (and perhaps Brazil if that has been added as well).
If you use those other platforms you will see the fee split between them and the fee will still roughly total $39.99 (it will not be exact due to currency conversion)
