Amazon FBA switching boxes sent and digging our shipment performance for a box thats not even ours!

A few weeks ago we shipped a small 5 box shipment to Amazon FBA to replenish low inventory. 4 of the 5 boxes checked in no problem and we waited for the 5th.... A week later we notice a shipment performance notification for an over weight box and our ability to ship to Amazon revoked!!!
Upon further investigation we noticed that it was the 5th missing box from our 5 box shipment. We are very careful to never go over the 50lb limit on boxes shipped to FBA so we knew this must be some sort of mistake as we've had this same "box overweight" issue falsely accused of us in the past. Because of this we now photograph all boxes and labels leaving our warehouse to prevent this problem and I'm glad we did in this case!
Amazon uploaded this photo of the 83.28lb- 20x20x23.60" box they received and claim is ours. It's not! We use 16x12x12 boxes with graphic printed on the outside and in the case of this box it weighed 45lbs which is corroborated by UPS tracking when it was checked in by UPS as delivered at Amazon!



What's even more strange is that the FBA label from our original box matches the label on the overweight box that Amazon sent us images of! However this new mystery box is missing the UPS label, box type, and tape type used on our box. Almost like Amazon switched our box and label once it arrived to Amazon and are now accusing us of making the mistake!
Will someone at Amazon please look into this flagrant error on Amazon's part and fix it immediately! This is so wildly unacceptable!
